The main route: contact with the mouth and saliva

According to the World Health Organization, HSV-1 is mainly transmitted by contact with the virus in sores, saliva, or the skin surfaces in and around the mouth. In practice, that means it commonly passes through:

  • Kissing, or other mouth-to-skin contact
  • Sharing items that touch the mouth — cups, cutlery, water bottles, lip balm, toothbrushes
  • Oral contact during a cold-sore outbreak, when the risk is highest

Most people first catch HSV-1 in childhood, often from a well-meaning kiss from a family member who carries the virus.

Oral-to-genital transmission

HSV-1 from the mouth can be passed to the genitals of a partner through oral contact, causing genital herpes. This is why HSV-1 — historically thought of as the "oral" herpes virus — now accounts for a substantial share of genital herpes infections.

"Silent" spread: no sore needed

HSV-1 can be passed on even when the carrier has no visible sores and feels completely well. This is called asymptomatic shedding — the virus is present on the skin or in saliva at times without any outbreak. The risk is greatest during an active outbreak, but it is never truly zero. This is a key reason the virus is so widespread, and why "they didn't have a cold sore" doesn't rule out transmission.

Transmission to newborns

A newborn can catch HSV during birth or shortly afterwards — including from an adult's cold sore through a kiss. Because a baby's immune system is immature, neonatal herpes can become severe very quickly. It is uncommon, but it is one of the most dangerous forms of HSV infection, which is why no one should kiss a newborn while they have a cold sore. This is covered further on the Serious Risks page.

What does not spread it

HSV-1 is fragile outside the body, so casual, non-intimate contact carries little risk. It is not meaningfully spread by:

  • Toilet seats, swimming pools or hot tubs
  • Being in the same room, or through the air like a cold or flu
  • Brief, everyday contact that doesn't involve saliva or a sore

Why it stays for life

After the first infection, HSV-1 travels along nerves and settles in a nerve cluster (for oral HSV-1, the trigeminal ganglion near the face), where it lies dormant. The immune system keeps it in check but cannot remove it, so it can reactivate later. This lifelong latency — not any failure of hygiene — is why the infection is permanent and why prevention focuses on reducing transmission rather than "curing" carriers.
